Do you have some more specific references to tickets with patches that
are being ignored? Its possible they were submitted to the wrong
person, or a module that still has an outdated maintainer reference
(IE: nobody actively involved). Or it could be that a person who
submitted a patch does not have a valid CLA on file (or they did not
provide enough information to link them to a CLA for verification)

As for avoiding duplication of work... anything being actively worked
on should have an open ticket in Trac and be assigned to the person
working on it...

As for the rest, general observations are a bit difficult to comment
on. We use trac to track working projects and changes, thats about the
extent to any comment that can be made on those general statements.
